I think the 787 is going to be a game changer for the aviation world. Airlines like Etihad, Emirates, Singapore, and most other major airlines (with the exception of US carriers) have an incredible hard and soft product. This includes chauffeur service, designated check in and security screening, spas in the terminals, world class food, high end wine, attentive flight stewardess, etc, but the plane itself has not evolved much. Most planes, whether in first class or coach class have small windows, are pressurized at a higher altitude, and feature annoying lighting among other things. The new 787 has larger windows, better lighting, is pressurized at a lower attitude, and are less noisy. I recently flew on a 737 with the new Sky Interior and that made a noticeable difference in the flight. I am still trying to get a flight on a 787, but am waiting for it to start international routes on United so I can further see how this new aircraft makes air travel more comfortable.

None of the delivered 787's have a 3 class cabin on any airline and only a handful of the 2 class cabin planes even feature full flat bed seats.

Virgin America upgraded me to First class on a very short 2hr flight because it was my birthday and the first class was empty. Seats were nice, but by the time the seat belt sign came off, 15 minute later the sign came back on for landing lol.

Domestic on US carriers is not that great, just a bigger seat, "more attentive" flight attendant, and early boarding. There is rarely a meal served and when there is a meal its just bad microwaved food. Most snacks and drinks are the same as coach just free (i.e. chips, beer, cookies)

International on US carriers is pretty good, service is poor in-comparison to European and Asian based airlines. Food starts to get better, more drink selection, seats have more adjustment and space, lots of seat level storage, and all the small aspects start to make a difference (i.e. less passengers per lavatory, smaller ratio of crew to passengers)

International on a European and Asian is very impressive, every detail has been considered. Top of the line everything and the staff will do just about anything to make sure your flight is pleasant and enjoyable.

I usually get a complementary upgrade for domestic, although sometimes I have to pay for an upgrade. International I usually wait for premium fares to be offered at a reasonable price if my schedule is flexible. If it is urgent I use miles or a upgrade on a close to full fare coach ticket.

However, you are right that a lot of first class tickets cost "stupid money." I was talking to a friend last week who told me his company had just bought Delta business class tickets to Amsterdam for $11K a piece, which is very high and probably could have gotten first class on a better airline for that price. Some companies have high level executives that need to travel far distances and a 14 hour flight costing $15K makes sense. Most people that are leisurely flying full fare first class international fly privately domestic and the plane they own can't go that far or would not be comfortable for such a distance and first class is comparable.
