What should I start with for my R8 V10?
Would like to start with cosmetics, but where?

naw cosmetics is for newbs. you're not a newb are you? what you want to do is call Kevin at Underground Racing, and add a little oompf to your car

Happiness is the only thing worth pursuing.

Hey there!
The R8 V10 is a great car out of the Box... What we would recommend is doing a set of 20" HRE or 20" Modulare Wheels and then add the Prior Design Aero Package, and top it off with the AWE or BBi Exhaust system

Visually, wheels and ride height are the two most striking things about a car. Don't go too big on the wheels, pick a finish that compliments the color of your car, and get some tires that fit the wheels and car properly. Lower it, but don't slam it like a '63 Impala on bags. Everything in moderation, IMO. My personal taste is for single piece wheels, others like multi-piece. Both look good.
This will set the tone for the car, then add stuff spoilers, splitters, carbon trinkets, etc. to taste.
